PRO TEM HEALTH INSURANCE PLAN

We know that many law office professionals would love the freedom and flexibility of working temporary assignments but need benefits—such as health insurance—that are normally offered only to full-time workers.

 

That’s why Pro Tem is proud to offer medical insurance benefits through the Sharp Health Plan to all our eligible employees ninety (90) days after their first day of work for Pro Tem.

 

If, after ninety days, you have worked an average of 30 hours per week since your first day of work you are eligible for health insurance coverage at no monthly cost to you.

 

Once enrolled, if you continue to work an average of 30 hours per week (120 hours in a 4-week period), your insurance premium each month is completely paid by Pro Tem!

 

If you work fewer than 120 hours during the previous four weeks, your next monthly premium is prorated (i.e., you pay a portion of the premium). For example, if you work only 60 hours during the previous four weeks (half the required 120 hours) you pay half the remaining premium amount. If you work no hours one month, you pay the entire cost of the following month’s individual premium: $326.30.

 

You may also choose to enroll other family members at your own expense:

            Employee’s child(ren)         $311.00

            Employee’s spouse             $404.92

            Employee’s family               $713.51

There are also nominal co-pay charges for doctor’s office visits, prescription drugs, eye exams, and outpatient mental health visits.
